What gives coffee a caramel taste?

Sugar caramelization during a medium roast. The bean sugars break down and rebuild into sweet, buttery compounds. It is real roast chemistry, not added flavoring. Latin American beans at medium roast carry it most clearly.

Is caramel coffee sweet enough to skip sugar?

Many people find it is. The sweetness is built into the roast, so a clean caramel-forward medium can taste sweet black. It will not be sugary like a dessert, but the perceived sweetness is enough that adding sugar often buries the note you paid for.
